Student Preferred Name

Elkin City Schools recognizes that a student may prefer to use a first name other than their legal name to identify themselves. A preferred name is the name that a student wishes to be known by. ECS will allow a parent/guardian to request a preferred first name for their child that will display on class rosters, gradebooks, yearbooks, and various reports. The student’s legal name will still be utilized on all official legal documents (i.e. transcript, diploma.)  
 
The following form is to be completed by the parent/guardian and returned to the student's school.
